Mom baffled after reading the health questionnaire her daughter got in her school: 'That's a red flag'

Lauren–who goes by @lauren1xoxo on TikTok–shared a video highlighting how her 13-year-old daughter received a health questionnaire that asked invasive questions. The video has garnered over 6 million views and 33.2K comments on the platform.

She starts the video by stating how the questionnaire seemed weird to her. The mother showcases how it began with standard medical questions, such as whether the student has any allergies or has asthma, diabetes or seizures. But then it got into more personal topics, such as the nature of her birth and whether it was a normal pregnancy. The questionnaire wanted the mother to share all the intimate details of the procedure and everything that followed.

In fact, the questions kept getting more intrusive with the school, somehow requiring her to let them know how old her daughter was when she started speaking words and putting them together to form coherent sentences. Another question was, "Do you feel that they were on par with the development of other children?" The woman questions, "Why? Why are you asking this? It's halfway through the year. She hasn't signed up for any sports or clubs or PE or anything like that that would warrant a new health questionnaire."

The mother revealed that the school did give out a standard health questionnaire form where parents shared important medical information. The woman says, "I feel like I have never filled out anything like this." She speculates that preschool children could be asked such questions when they start out, but the same would not apply to teenagers. People were quite shocked to hear about the details in the questionnaire and shared their thoughts in the comment section. @theblonde_cowgirl said, "That's a red flag. Lauren was suspicious and posted a follow-up video in which she decided to call the health services coordinator for her school district for some clarification. She got on the call and began questioning her, and the woman's answer proved quite amusing, with her saying that she had never seen such a form. The coordinator then tried to get her to call the principal to sort out the issue, but the woman pointed out that they would not be responsible for sending such a form. She concluded the video by sharing how she would continue to dig deeper and get to the bottom of who decided to send out these medical questionnaires.
